We are currently searching for a LVN to work at the Naval Health Clinics in Hawaii.
Collaborate and integrate with members of the Medical Home Port (MHP) Team, other departments, and directorates to achieve optimal care for patients in the MHP model.
Actively participate in MHP Team huddles. Promote effective communication with staff, patients, families, and between other departments.
Provide patients and/or family members information building upon other MHP Team guidance and recommendations. Instruct patients and/or family members in self-care using clinic resources.
Assist the Team RN in ensuring efficient MHP Team workflow.
Schedule patient appointments.
Prepare patient electronic health record and other appropriate forms prior to patient’s appointment.
Participate with other health professionals from the command and community supporting the department, nursing, and command’s Chain of Command.
