Cape Fox Federal Intergrators is seeking an (APRN Midwife OB/GYN) to work full time providing care to our deserving Army and their families at Ft. Stewart, GA.

This is for a future position to start in the coming weeks.

Duties
Provides independent preventive, therapeutic, and comprehensive obstetric and gynecologic care. Collaborates with Obstetricians and Gynecologists when caring for women with obstetric, gynecologic, and general medical complications in both the ambulatory and inpatient settings. Makes decisions regarding medical and nursing care of patients and directs care, co-manages, or refers as appropriate. Plans and coordinates care given with other professional providers and agencies involved in patient and family care. Performs accurate history and physical examination on obstetric, gynecologic, and neonatal patients, making critical evaluation of patient's status, establishing diagnoses, recording examination findings and evaluation, and planning and/or providing follow-up care. Performs and/or requests laboratory, radiologic and other diagnostic and screening tests, interprets results, assesses specific problems/complications as appropriate, and provides follow-up as needed. Discriminates between normal and abnormal assessments; manages, co-manages, and/or refers appropriately; informs patient/family; and initiates appropriate nursing intervention/medical consultation. Manages labor to include initial evaluation/hospital admission, ongoing assessment of maternal and fetal status, labor progress, need for intervention/consultations, and patient/family education and support. Provides inpatient obstetrical care including labor management and vaginal delivery. Starts intravenous infusions; selects and administers appropriate analgesia and anesthesia according to protocols; applies external and internal fetal monitoring equipment; performs local and pudendal anesthesia; performs cervical inspections; performs and repairs episiotomies; repairs lacerations associated with childbirth 1st and 2 degree; records the labor and delivery, and signs and completes birth certificates. Evaluates and manages immediate obstetric complications. Initiates and conducts emergency lifesaving procedures until patient management can be transferred to the consulting physician. Acts as the first assistant to the obstetrician during emergency caesarean sections, obstetric and gynecologic surgery. Provides postpartum care using a family centered approach. Evaluates and manages the normal postpartum period to include determining hospital disposition, providing discharge counseling, completing necessary records, and conducting clinic follow-up. Provides well woman care to include family planning, counseling and the prescription of contraceptives. Manages the care of the normal antepartum woman through routine assessment of maternal and fetal status, maternal and family counseling and support, and teaching prenatal classes to include orientation, labor preparation and breast-feeding. Provides immediate newborn care, initial physical assessment of the newborn upon delivery, evaluation, and resuscitation as needed. Manages care of normal newborn including nutrition, elimination and activity. Knowledge of nursing management and care of mothers and families throughout the maternity cycle, including normal deliveries. Ability to recognize deviations from normal and initiate the appropriate nursing or medical intervention to safeguard the wellbeing of the mother and her baby. Knowledge required and ability to make independent decisions to implement or alter medical/nursing plan as needed for OB/GYN patients managed, e.g., to prevent and/or treat emergencies and complications. Perform obstetrical physical examination and review and evaluate findings and laboratory data, present those findings with the patient/family, and consult with physician when indicated. Teach and counsel women and their families during pregnancy and after childbirth to assess emotional adjustment to parenthood. Collect, organize, record, and communicate data relevant to OB/GYN health assessments including a detailed family and medical history, a physical examination, and selected laboratory tests. Knowledge of hospital policies and procedures and proper maintenance of records including labor records, progress notes, and discharge summaries. Knowledge to prescribe treatments and medications as outlined in protocols, and to make referrals to other health care disciplines when indicated. Specialized skills to perform anesthesia techniques such as local and pudendal blocks, operative techniques such as episiotomies and suturing, and to assist the physician with caesarean sections, obstetric and gynecologic surgeries. Maintains a high standard of professional care through academic preparation, continuing education and consistent competency by adhering to the standards for the practice of nurse midwifery established by the American College of Nurse Midwives. Must maintain current registered nurse license and CNM, Neonatal Resuscitation program (NRP), BLS. Knowledge of family planning and contraceptive methods and techniques. Knowledge and skill to recognize obstetrical and neonatal emergencies, initiate emergency lifesaving procedures, apply appropriate nursing intervention, and refer management to the physician. Knowledge and ability to apply, operate, and interpret electronic fetal monitors. The Nurse Midwife is part of the OB/GYN health care team and performs under general supervision. Assignments are established by the nurse midwifery service protocols. Essentially normal mothers and babies are assigned. Is self-directed and functions independently as long as progress meets criteria accepted as normal. Works independently in emergency situations prior to the arrival of physician. Transfers to medical management any woman or baby who develops complications which, in the judgment of the nurse midwife or physician, are best managed by the physician. Work is reviewed by peer review and physician evaluations for effectiveness in meeting program requirements and for quality of care provided. Works within a conceptually clear framework of practice protocols that broadly define the scope of practice. Individual assessments often reveal variables that cannot all be covered in guidelines. Deviations from normal require self-direction and judgment in planning, directing, consulting with the physician, referring, and providing care. Judgment is required to recognize deviations from criteria established as normal. The Nurse Midwife practice covers a wide range of independent and collaborative health services including assessment, planning, direction, coordination, evaluation, and providing direct and support care to women and their families. The work involves counseling, teaching, coordinating services, and participating with other health care disciplines in planning, interpreting, providing and evaluating care. The Nurse Midwife is a primary provider of OB/GYN health care in the military community. The assignment encompasses antepartum, intrapartum, postpartum, neonatal, gynecologic care, and intraconceptual nursing care in a family centered context. The incumbent must make decisions regarding what must be done to provide quality health care. The work involves coordination with other health care disciplines, involvement with the medical facility committee, and other administrative activities. Specialized skills are required to accomplish operative procedures, and to exercise appropriate measures in life threatening situations. The practice requires identifying and documenting new information and new approaches, guidelines, and acquiring more advanced knowledge and skills as new information and techniques frequently emerge in the OB/GYN field. The purpose of the work is to provide a broad range of assessment, planning, provision, teaching and evaluation aspects of care for obstetric, gynecologic, and neonatal patients. These aspects are provided in both ambulatory and inpatient settings and involve the continuum of services from routine to emergent care. The Nurse Midwife manages normal pregnancies collaborates care for high risks patients, performs deliveries, provides immediate care to infants at birth, provides appropriate measures in lift threatening situations, assists on operative procedures, and provides postpartum and intraconceptual care and counseling. The work has a profound effect on the physical and emotional well-being of a substantial number of patients and their families, and on the continuity and quality of care available to the military community through this medical facility. Contacts are made with patients, their families, physicians, nurses, and other health care professionals, unit military personnel, community groups, assistance groups, and other health care facilities. With patients and families, the purpose is to provide services, assessment, treatment, counseling, education and guidance, health care promotion, and to influence and motivate patients to care for themselves, follow medical orders, etc. Contact with unit personnel is to provide education and guidance regarding patient/families health care status and needs. Contacts with physicians are for collaboration and receiving or rendering consultation. Contacts with other health professionals and medical personnel are to exchange information, consultation, education, and planning and coordinating care. Contact with other providers and groups is to exchange information, give advice, and to request services. Work requires prolonged periods of standing, occasional moderate lifting required for the shifting or moving of patients, physical characteristics and abilities such as agility and physical dexterity, and prolonged duty periods. Work is performed in a hospital setting where exposed to infectious disease, bodily fluids, blood borne pathogens, and anesthetic agents is common. Precautions are routine for nearly all situations. May be required to use protective clothing or gear.
Utilize electronic hospital information system s to check-in patients, document inpatient/outpatient evaluations, follow up and treatment notes in accordance with PT section documentation guidelines and policies. Ensure encounters are closed within 72 hours. Contact s r eferring providers to clarify consult requests or order instructions. Duties require providing manual assistance in lifting and positioning patients, and extensive periods of standing and walking throughout the clinic. Midwife duties are performed in an inpatient or outpatient setting in an environment involving everyday risks or discomforts requiring normal safety precautions. The performance target for the Certified Nurse Midwife is at least 286 Provider Aggregate Work RVU's /month with an associated "in-clinic" time allocation equal to or greater than .90 FTE.

Job Requirements

Shall have graduated from an accredited Midwifery Program in the United States and hold a valid state license as a Certified Nurse Midwife with National certification through AMCB. Shall have a minimum of one (1) year of experience, within the past two years, working as a Certified Nurse Midwife. Shall hold an active unrestricted RN license in any of the 50 US States. Maintains continuing education and training requirements not on paid government time. Is a current US Citizen.
