Job Description
Eastern State Hospital, located in Williamsburg, Virginia, was founded in 1773, with a well-intentional emphasis on community focused mental health care. This state of the art campus, situated on 500 acres, consists of two patient care buildings with 300 patients, and a highly skilled staff of over 700. Eastern State Hospital is part of the Department of Behavioral Health and Developmental Services ( DBHDS ) system, and is accredited by the Joint Commission.


ESH invites applicants for the position of Treatment Team Coordinator.


Reporting directly to the Clinical Director, the Treatment Team Coordinator coordinates the development of comprehensive and individualized treatment plans that are inclusive of mental health, substance use, and medical/nursing care needs. As a member of the Treatment Team, the Treatment Care Coordinator monitors and ensures the accurate and timely completion of treatment care plans and all required documentation relevant to treatments and services that are consistent with individually planned, sound, and therapeutic interventions that conform to current acceptable professional practice.

Minimum Qualifications
• Demonstrate knowledge of clinical documentation standards and an ability to utilize and interpret relevant resources, including but not limited to DSM -V, PDR and Physician's Drug Reference Book, TJC , and CMS standards of care, etc.

• Ability to interpret documentation in medical records, other reports, and requirements as they relate to clinical practice and make recommendations for improvement.

• Must have experience with evidence and evidence-based psychosocial treatments for common mental disorders and the ability to utilize stages of change, motivational interviewing techniques, and shared decision-making tools to promote the development of person-centered treatment plans.

• Must have experience working on an interdisciplinary team and experience with treatment care plan development.

• Must be able to perform assigned duties as a productive member of an interdisciplinary team and facilitate positive relationships within the tram as well as with the individuals receiving services, authorized representatives, clinical and administrative employees throughout the system.

• Strong written and oral communication skills are essential to this position.

• Must also be able to positively influence the work of other team members and perform other duties as assigned effectively and efficiently.

• Must be able to represent ESH in a professional and positive manner at all times.

• Extensive experience working in the psychiatric hospital.

• Demonstrated knowledge of psychiatric treatment pertaining to the adult and geriatric age groups.

• Familiarity and ability to compose a Master Treatment Plan inclusive of goals, objectives and interventions specific to each patient's individualized needs.

• Demonstrated knowledge of psychiatric disorders, substance abuse disorders, and physical conditions.

• Experience developing and evaluating non-pharmacological interventions to address biopsychosocial needs of patients with varying degrees of acuity and complexity.

• Experience using Microsoft Office programs, including Word, Excel, and Outlook.

• Ability to use Electronic Health Records.

Preferred Qualifications
Master's level degree in the field of counseling, social work, nursing or rehabilitation services and demonstrated knowledge of psychiatric treatment pertaining to the adult and geriatric age groups is preferred.
