Description:
Provides home health care to Kaiser Permanente's Home Health patients in coordination w/ physician orders & nursing interventions. Maintains professional, clinical, technical, & interpersonal skills to provide best possible service.

Essential Functions:
- Establishes & coordinates plan of care for patients assessing educational, equipment, skilled & nonskilled services & needs. Maintains productivity levels per agency standard.
- Evaluates patients, families, & home environment on admission & subsequent home visits. Determines if home environment is safe & appropriate for provision of home health services. Assesses patient's physical & psychosocial needs. Develops &/or revises an appropriate plan of care. Communicates & coordinates plan of care w/ physician(s), patients & family, & other health care providers. Implements care, as ordered. Identifies problems accurately. Establishes reasonable/measurable goals. Establishes discharge planning in conjunction w/ the care planning process. Prepares & delivers medical supplies & orders equipment necessary for provision of prescribed care/service.
- Provides comprehensive nursing care as prescribed by patient's physician & in compliance w/ Home Health Services established standards, policies, & procedures. Evaluates patient's response to identified interventions. Reviews & interprets laboratory values. Demonstrates knowledge & skill in providing wound care. Provides all care according to established, written plan of care. Modifies/updates/changes plan of care when indicated by patient's condition &/or physician orders. Obtains physician orders as required by State & Federal laws. Initiates appropriate preventative & rehabilitative nursing procedures.
- Recognizes & interprets significant physiological & behavioral changes in patient's condition. Reports findings appropriately to physician, Home Health Manager &/or other individuals involved in care of patient. Demonstrates critical thinking such as problem-solving, reflective practice, clinical reasoning, prioritizing, etc. Determines in conjunction w/ physician, when patient care needs to be shifted into another setting. Re-evaluates patient's nursing needs regularly when licensed practical nurse furnishes services.
- Instructs patient, family, & other caregivers on nursing & aide-related care to promote independence & self-care.
- Provides case management, as caseload assigned. Coordinates all necessary services to be provided to patients. Integrates all services into plan of care.
- Documents all care provided to patients & plans pertaining to care according to Home Health Services standards, federal & state regulation(s). Assures that medical record is accurate, legible, & complete; & completes accurate daily visit log. Completes assessments & documentation per State & Federal law (e.g. OASIS) for all applicable patients assigned. Submits documentation in timelines specified by Home Health Standards, Federal & State regulation(s).
- Develops area of clinical expertise w/in Home Health Nursing, & provides instruction/supervision in area to others in agency, as requested. Provides guidance & supervision to licensed practical nurse. Supervises care provided by Home Care Aides (HCA). Prepares HCA assignments. Coordinates patient care. Makes HCA evaluation visits, & provides feedback to HCA. Documents HCA supervision.
- Plans for & participates in professional development activities to keep current of trends in home health nursing &/or clinical nursing skills.
- Participates in multidisciplinary team conferences (formal & informal) to coordinate, discuss, & evaluate services provided to patient. Communicates w/ other Home Health personnel, other departments & w/ community resources to coordinate care & services for Home Health Services patients.
- Participates in Home Health Services functions such as staff meetings, staff in-services, & committees, as assigned. Participates in orientation of new personnel to Home Health Services & precepts students as assigned.
- Participates in performance improvement. Identifies & presents improvement opportunities to organization & implements Quality Improvement process(es). Identifies & reports infections of patients. Assists w/ development & implementation of department's annual work plan. Performs clinical record audits, as assigned. Maintains educational record & provides evidence of competency.
- Prepares weekly schedule of patient visits for self, & licensed practical nurse, & keeps Referral Center apprised of schedule changes & openings for new admissions & other added visits. Rotates weekend, holiday & after hour coverage w/ other Home Health nurses, as required.
- Notifies Home Health Manager &/or designee of unusual circumstances or abnormal conditions which might result in schedule change for self or other employees. Reports problems, incidents &/or concerns to Home Health Manager in timely manner.
- Follows all applicable laws, rules & recommendations for safe driving while on duty.
- Provides verification of current licensure/certification.
- Maintains, uses, & responds to (as applicable): voicemail, pager, e-mail, & computer.
- May perform patient care to the extent necessary to maintain clinical expertise, competency & licensing necessary to fulfill job responsibilities & to direct the provision of care on the unit.
- Provides direct patient care on an as needed basis. Provides services that are w/in scope of license & in compliance w/ all legal, regulatory, & policy requirements relevant to clinical role performed.
- Incorporates the KP Nursing Vision, Model & Values through out their Nursing Practice.

Basic Qualifications:
Experience
- Two (2) years medical/surgical nursing experience, including experience or successful completion of training program (pre-hire, post-hire, or on-the-job) in area of specialty, if applicable.
Education
- Associate's degree from accredited school of nursing.
License, Certification, Registration
- Valid driver's license.
- Valid Hawaii RN license (must meet education requirement(s) for Hawaii State licensure).
- Current BLS for Healthcare Provider CPR or CPR/AED for the Professional Rescuer certification.

Additional Requirements:
- Demonstrated knowledge of and skill in oral communication, written communication, interpersonal relations, customer service, problem solving, systems thinking, decision making, adaptability, and change management.
- Talking to co-workers, customers, outside vendors, and on the telephone.
- Reading, writing, speaking and understanding English.
- Training/giving and receiving instructions.
- Mathematical ability, attention to detail (e.g., organization, prioritization, proofing), concentration, and alertness.

Preferred Qualifications:
- Home Health agency (Medicare certified agency) experience.

- Bachelor's degree in nursing.
- Certified Home Health Nurse by the American Nurses Association (ANA).
- Demonstrated knowledge of venipuncture and IV therapy, antibiotic therapy, respiratory therapy, patient assessment (physical and psych/social), hyperalimentation and TPN, colostomy and ileostomy care, and catherization (male and female), bowel and bladder training, wound care, diabetic care, transfer skills, care of terminally ill, and patient teaching.
